Transitional Care
Stafford Healthcare offers post-acute services for patients after an acute care hospital stay and prior to returning home. These services provide 24 hour nursing care and include complex medical management, respiratory treatment management, post surgical care, tube feeding, wound care, IV therapy, infectious disease, total parental nutrition and other daily skilled nursing services. Services also include physical, occupational and speech therapy programs.
